Community Care Access Centre

Community Care Access Centres (CCAC) are local organizations established in 1996 by the Ministry of Health and Long-Term Care to provide Ontarians with better access to government-funded community-based health services, and to facilitate the placement process into long-term care homes.

Waterloo Wellington CCAC is one of 14 CCACs in Ontario. CCAC works closely with community health care partners - from Local Health Integration Network, to physicians, hospitals, Family Health Teams, health care agencies and community support services - to enhance access to community-based health services for people across the province.
